Shift 1 — First Night Expectations
Shift 1 functions as an on-the-job tutorial wrapped in horror atmosphere. Patient arrivals are slow, the game highlights obvious anomaly tells like Three Eyes or Sharp Teeth, and environmental enemies almost never spawn. Use this night to memorize desk layout: Shutter button, photo camera mount, CCTV switcher, and paperwork clipboard slot.
Take photos of every visitor even when tells seem obvious. Photo anomalies hide differences — extra limbs, wrong fur patterns, cursed backgrounds — that in-person inspection misses. Compare the snapshot to the live patient before opening the Shutter. Shift 1 rarely punishes a wrong rejection, but building the habit prevents Shift 4 disasters.
Assign roles even in pub lobbies. Whoever touches the desk first should stay there the whole Shift. Others learn the path from check-in to Room 1 DNA station. Voice callouts like "clean admit" or "reject cam mismatch" reduce confusion when Shift 2 speeds up slightly.
Shifts 2 and 3 — Building Consistency
Shifts 2 and 3 introduce Hollow Eyes, missing appointment paperwork, and occasional reverse speech audio cues without yet spawning Ambulances. Anomaly rate increases modestly — expect one suspicious patient every few legitimate admits. Never skip CCTV because early nights still spawn camera-only tells invisible at the window.
Start buying Coffee from the Shop after Shift 1 if you earned Animal Coins. One stack per player is enough for these nights. Intern Class is free and grants plus ten starting Sanity, making it the best pick until you unlock Doctor or Surgeon. Avoid Psychologist until you understand doubled Sanity swings.
Treatment flow for real patients: Room 1 DNA swab, match result to the treatment item chart, apply meds, discharge. Wrong items kill — read the DNA output carefully. Shift 3 occasionally routes patients to Room 6 X-Ray; send a runner left while desk stays staffed.
Anomalies You See Before Shift 4
Visual tells dominate early bands: Three Eyes, Hollow Eyes, Sharp Teeth, twitching animations, and shadow figures visible on CCTV. Photo anomalies add mismatched reflections and cursed photo backgrounds. Paperwork anomalies show wrong appointment times or species names that do not match the visitor.
Audio anomalies use reverse speech or distorted barks. Turn volume up and listen before admitting. Mimic patients copy previous real visitors — photo comparison catches them when memory alone fails.
Reject protocol: close Shutter, log the rejection mentally, and move on. Do not open for "one more look" — admitted anomalies walk inside and convert later. Early Shifts give forgiving conversion timers, but the habit must be absolute before mid-game.
Shop Spending for Shifts 1–3
Priority one is Coffee for Sanity insurance. Priority two is saving coins toward Doctor or Surgeon unlock rather than spamming consumables. Priority three is a single Fire Extinguisher once you see the fire Event tutorial — usually Shift 2 or 3.
Skip Camera Upgrades until Shift 7 approaches. Skip weapons unless you accidentally admitted an anomaly and need emergency Skinwalker tools. Secret Agent and Security are late purchases for most F2P grinders.

Sanity Management in Early Shifts
Sanity drops from jumpscares when anomalies approach the window, from letting patients wait too long, and from witnessing environmental scares. Early nights drain slowly enough that Coffee alone stabilizes you. Drink before you hit critical — sub-twenty Sanity triggers visual distortion that makes photo checks harder.
Doctor Class restores one Sanity when you successfully heal a patient — strong value once unlocked. Surgeon adds movement speed after heals, helping runners clear Room queues faster. Both outperform Nurse's extra inventory slot for beginners who forget to stock items.
Co-op Sanity is independent per player. Do not assume your partner's Coffee heals you. Share consumables by dropping or calling out Shop purchases between Shifts.
Preparing for Shift 4
Finish Shift 3 with at least one healing Class unlocked, four Coffee per player stocked, and every teammate confident on Shutter protocol. Shift 4 adds Ambulance events that dump multiple critical patients at once while anomalies still queue at the window — the first real multitask crunch.
Review left-wing rooms before Shift 4: Room 6 X-Ray sequence, Room 7 heart minigame, Room 8 surgery timer. Early Shifts may skip these, but Ambulance patients often need advanced care immediately.
If your lobby struggled on Shift 3, repeat early bands in a new run to bank coins rather than pushing into Ambulance nights under-geared. Animal Hospital rewards clean fundamentals more than brute-forcing shift numbers.
Shift 1–3 Preparation Checklist
Before starting Shift 1 in a fresh lobby, confirm all players selected Intern or saved Class choice, understand who owns desk, and agree on voice or text channel for anomaly callouts. Load into clinic, walk the path from check-in window to Room 1 DNA station once before the night timer starts — orientation prevents panic sprinting when first real patient arrives.
Shift 2 should begin with at least one Coffee purchased if Shift 1 earned twenty or more Animal Coins. If coins are tight, replay Shift 1 in a new pub lobby rather than entering Shift 2 broke — the economic snowball matters more than ego about shift number display.
Shift 3 is your certification exam for mid-game: can you reject a mimic using photo comparison under mild time pressure while a teammate completes DNA on a legitimate dog or cat patient? Failure here means deliberate practice, not pushing forward into Ambulance nights unprepared.
Events During Early Shifts
Fire Events and fainting patients appear in Shifts 2–3 as teaching moments. Carry Fire Extinguisher by Shift 3 inter-Shift Shop if you witnessed fire once. Fainting patients need correct stabilizer item from treatment chart — desk player reads DNA or symptom UI aloud so runner does not guess wrong med.
Barney Event can theoretically trigger early though most lobbies see Barney mid-run later. If Barney appears Shift 3, squad should pre-vote hide versus turn-in before engaging — hide grants second coffee machine valuable for entire remaining run.
Death Ritual rarely appears Shift 1–3; if it does, treat as bonus lore with Sanity cost — skip if team feels unprepared and focus on living patient queue instead.
