Anomaly frequency rises noticeably. Expect at least one suspicious visitor per Shift segment, sometimes back-to-back. Mimic patients copy recently treated animals, punishing autopilot admissions. Photo and CCTV checks become non-negotiable every single visitor.
Environmental enemies begin spawning: Black Bunny in corridors, Stalker outside windows, Eyeless in dark corners. These are not Skinwalkers — they drain Sanity or block paths. Medkits and movement discipline handle them; do not abandon desk for chase unless Float role is free.
Ambulance Event Protocol
When Ambulance audio triggers, critical patients spawn at the emergency bay. They bypass normal check-in — no anomaly test — but require fast routing to Room 7 heart monitor or Room 8 surgery. While runners sprint left, desk must keep rejecting window anomalies independently.
Assign Ambulance duty to your fastest Class — Paramedic with starting Speed Cola excels. Pre-position treatment items in left-wing storage during inter-Shift Shop. Running back to central inventory mid-Ambulance loses precious timer seconds.
If Ambulance stacks with Death Ritual or fire Events, prioritize living patient timers over ritual side objectives unless your team knows the ritual skip. Shift 4–6 wipes often come from choosing lore over survival.
Preventing Skinwalker Conversions
Admitted anomalies wander treatment rooms, then transform into Skinwalkers that kill patients and staff. Rejection at Shutter is the only clean prevention. Once inside, guns and tasers work but cost Sanity and time.
Shift 4–6 conversion timers are shorter than early nights. A missed mimic may transform within the same Shift. Security Class starts with Taser charges; Secret Agent starts with Gun ammo — consider one combat unlock if check-in mistakes persist while learning.
Wrong medicine on an anomaly kills it without score penalty. DNA still returns a result — if treatment fails repeatedly, suspect you admitted a hostile and close the Shutter on future similar profiles.
Shop Strategy for Shifts 4–6
Unlock Surgeon or Doctor before luxury items. Healing passives compound across Ambulance nights where runners take repeated Sanity hits from critical care minigames. Head Nurse is Robux-only premium — strong but not required.
Stock two Coffee per player minimum entering Shift 5. Buy Camera Upgrade if approaching Shift 7 in the same run — the upgrade persists for remaining nights and helps Hollow Eyes detection on CCTV.

Co-op Adjustments for Mid Shifts

Mid-Game Anomaly Mix
Shifts 4–6 introduce denser mimic and cursed photo frequency. Hollow Eyes appear on both live patient and CCTV simultaneously — desk must compare all three layers: in-person, photo print, camera feed. Missing any layer admits imposters.
