This tier list ranks Animal Hospital Classes by impact on Shift survival, Sanity economy, and co-op value — not cosmetic flair. Rankings reflect community testing across Shift bands 1–10+ as of June 2026 from Animal Anomaly's launch build. Patches may move entries; we update when verified changes ship on the Roblox game page.
S-tier Classes change run outcomes noticeably. A-tier strong alternatives with narrower use cases. B-tier situational picks that excel in one role. C-tier niche or high-risk choices like Psychologist that double all Sanity effects — powerful in expert hands, punishing for beginners.
Robux Classes are labeled separately. Head Nurse competes with top F2P picks but costs real money. Secret Agent and Security are partially Robux or coin hybrids depending on shop rotation — check in-game Class Shop for current pricing.
We weight desk-heavy Shifts, Ambulance nights, and Skinwalker cleanup equally because a Class that only helps one phase falls below generalist healers. Tier placement assumes four-player co-op with voice; solo rankings shift slightly toward Surgeon self-sustain.
S-Tier Classes
Surgeon earns S-tier for F2P players because healing patients restores Sanity and grants temporary movement speed — critical for Ambulance chains in Shifts 4–9. Every successful treatment becomes a micro-recovery, reducing Coffee dependency. Speed burst helps Room 8 surgery rushes and fire Event sprints.
Doctor sits alongside Surgeon with plus one Sanity per heal without speed bonus. Pure healers who stay on DNA and cardiac rooms love Doctor for steady meter growth. In four-stacks, one Surgeon runner plus one Doctor desk backup covers both mobility and sustain.
Head Nurse is S-tier on Robux accounts. Passive bonuses mirror premium support — extra inventory flexibility and strong heal-adjacent benefits that accelerate mid-game Shop efficiency. Not required to clear Shift 10+, but noticeably smooths progression for paying players.
Neither Surgeon nor Doctor prevents anomalies at the Shutter. S-tier means fewer Sanity wipes mid-Shift, not permission to skip CCTV. Pair S-tier healers with a disciplined check-in lead every night.
A-Tier Classes
Paramedic starts with Speed Cola, making Ambulance Events and left-wing sprinting easier from Shift 4 onward. Less Sanity sustain than Surgeon but faster Event resolution — ideal third slot in coordinated teams.
Intern is free with plus ten starting Sanity every Shift. Best Shift 1–3 pick and viable filler until coin unlocks. Falls off when heal passives matter more than buffer Sanity.

B-Tier and C-Tier Classes

Classes and the Animal Coin Economy
Class unlocks compete with Coffee, Camera Upgrades, and Fire Extinguishers for the same Animal Coins earned at inter-Shift payout. Robux Head Nurse does not change anomaly rules at the Shutter. Paying players still need desk discipline. If you already own Surgeon and Doctor on F2P coins, Head Nurse drops from S-tier appeal to luxury QoL — reasonable for supporters, not a power gate.
Psychologist remains C-tier in pub tier lists even though theoretical throughput exceeds Surgeon when a four-stack scripts Coffee chugs and heal chains perfectly. When Animal Anomaly patches Class passives, expect Surgeon speed duration or Doctor Sanity values to move first — those two dominate June 2026 clear rates in community logs. Shift Band Class Matrix
